Below are forms and documents and a description of why a student may need this form.

Advising Release Form

Students with administration advising holds should complete the

, save, and email to the Student Success Center for review.

Overload Permits

An overload permit is required for any student wishing to exceed the maximum amount of units allowed during the priority and open registration period.

Overload permits are available in:

  • Office of the Registrar - University Hall, Room 171
  • JHBC Student Success Center - Jack Brown Hall, Room 134

Overload classes cannot be added to your schedule until the first day of late registration, even if you have all three signatures.

Overload permit procedures:

  • Obtain an overload permit.
  • Complete the overload permit out in its entirety.
  • Obtain the signature of the instructor teaching the course.
  • Obtain the signature of the department chair of the course.
  • Submit the overload permit to the Dean, or Dean's designee in the College of the major for final approval. 
  • (Administration students, please see the Dean's designee - Jack Brown Hall, Room 134)
  • Once approved, submit the overload to the Office of the Registrar for course enrollment.

Overload requests cannot be submitted until the first day of late registration - check academic calendar for dates.

Petition to Drop After Census Date

This petition (PDF) is required when attempting to drop a course AFTER the census date (typically 3rd week of classes).

Approval will only be granted for serious or compelling reasons, and documentation is required. Poor performance or attendance is not considered a serious or compelling reason, and petitions citing this reason will be denied.

If the petition is approved the course is defined as a withdrawal and a grade of "W" will be posted on the student's record.

  • JHBC Student Success Center - Jack Brown Hall, Room 134

Petition to Drop After Census Procedures:

  • Obtain a petition to drop after census form.
  • Complete the petition in its entirety.
  • Gather required supporting documentation, and attach to the petition.
  • Obtain the signature of the instructor teaching the course.
  • Obtain the signature for the department chair of the course.
  • Submit the Petition to Drop After Census to the Dean, or Dean's designee in the College of the major for final approval. (Administration students, please see Dean's designee - Jack Brown Hall, Room 134)

Petition to Add a Course after Census Date

This petition to add a course (PDF) is required when a student needs to add a course after the censu date (typically 3rd week of classes).

Approval will only be granted for serious or compelling reasons, and documentation is required.

Petition to Add a Course after Census Date forms are also available in:

  • Upon approval, submit petition to the Office of the Registrat (University Hall, Room 171) for course enrollment.
